These alterations converge on immune–metabolic injury pathways and tissue injury programs within the renal allograft, promoting divergent clinical phenotypes such as recurrent urinary tract infections, chronic allograft dysfunction with interstitial fibrosis and tubular atrophy (IF/TA), and acute rejection.
