Inflammatory retinal detachment in patient after renal transplantation with systemic cmv and cryptococcosis

Abstract

Background: 46-years old female patient underwent renal transplantation in February'1997. In postoperative period acute rejection of the renal transplant was observed. This was a reason for administration of aggressive immunosuppressive therapy. Reactivation of systemic CMV and Cryptococcosis were adverse effects of this therapy.
Material/Methods: Observational case report. Ophthalmological examination, biochemical tests and eye ultrasonography were performed.
Results: Ophthalmologic examination revealed a local CMV retinitis of the left eye which regressed gradually. Ten months later exudative retinal detachment was found in the other part of retina, distant from the site previously observed CMV retinitis. Patient was treated systemicaly with Gancyclovir, Flukonazole and Amofotericin B. Inflammatory changes were resolved and chorioretinal scarrring developed as a result of therapy.
Conclusions: Exudative retinal detachment in patient with CMV retinitis, systemic CMV and Cryptococcosis probably had pluricausal mechanism.
Possible mechanisms of retinal detachment in this patient are discussed.
During 11 years follow-up no ophthalmological problems have been detected.
